add security guide covering threat model

New guides/security.md documents the trust boundary users own: the
database file, the SQL they run, and any loaded extension all run
with full BEAM process privileges. Covers the thread-safety model,
resource-lifecycle leaks from mis-ordered teardown, the deny-list
authorizer, SQL injection, the panic-freedom guarantee and its
destructor boundary, and the API_ARMOR safety net. Wired into the
docs extras so it renders on hexdocs.
