Update row when closing Comment modify modal

`CommentsController#update`/`#destroy`/`#create` only closed the modal
synchronously, relying on `Comment`'s async Action Cable broadcast to
update the visible row. When that broadcast didn't reach the
submitter's own tab in time, the row appeared unchanged (#4833).

Adds `CommentsController::RowStreams` to apply the row
prepend/update/remove synchronously in the same turbo_stream response
that closes the modal. Extracts `CommentsController::ModalForm` to
keep the controller under the `Metrics/ClassLength` limit.
