feat: add document links for module imports

Adds textDocument/documentLink support: the "at" path in `import module
namespace ... at "path"` becomes a clickable link to the resolved module
file, and string-literal arguments to fn:doc()/fn:collection() link to
the referenced file when it exists on disk.

Closes #118
