Reduce Azure Table v2 enumeration RTT overhead (#168)

Tighten the Azure Table v2 requirements, design, and validation specs so normal enumeration yields recognized block IDs from root-row query metadata without per-block continuation-row reads.

Update the v2 implementation to remove enumeration-time row-set re-reads while preserving explicit failure for malformed recognized root metadata and existing query retry behavior. Refresh the tests to prove enumeration stays query-metadata-only in the normal path.
