* Use direct entity lookups for Azure Table v2 point reads

Tighten the Azure Table v2 requirements, design, and validation specs so point reads must use direct entity-addressed lookups instead of filtered queries.

Update the reqwest backend to call the direct entity endpoint for known row addresses, preserve 404-to-None semantics, and add tests that prove point reads stay on the direct-read path across success, absence, and retry cases.

* Improve Azure Table v2 point-read decode diagnostics

Include the direct entity lookup PartitionKey and RowKey in the entity GET decode error so malformed payloads are easier to diagnose without dumping response bodies.
