test: resolve QT4 catalog module entries in the diagnostic pipeline

<module uri="..." file="..."/> catalog entries are the QT4 harness's own
namespace-to-file association, used when a query imports a module
without (or without a resolvable) "at" location hint. Previously these
never resolved at all, so the worker's `imports` map only ever held
builtins. Parsing and feeding them in lets XQST0059 catch the
modules-bad-ns case, and as a side effect fixes several pre-existing
false positives caused by catalog-declared symbols never resolving. It
also exposes a few unrelated pre-existing gaps (reserved-prefix
checking, circular-import detection, %private visibility) that were
previously masked by that same non-resolution.
