GH-251 - Flesh out WebMVC0/Connection.pod; document URL entities in SiteApp.pod

Connection.pod now covers url builders (site_path family), redirect
(raise_redirect), request parameters, location inspection (with a
worked example of all path-ish values), low-level mkurl family with
its argument caveat, env accessors (including the raw script_name
caveat), session and misc methods.

SiteApp.pod gains an ENTITY FUNCTIONS section for URL-related
entities, including the lowercase-vs-UPPERCASE convention and
a value table in the style of yatt-cookbook's script_name.md.
