docs(plugin): harden revdiff timeout fallback in review skills

when the launcher's blocking bash call hits the harness tool timeout, the
captured annotations end up only on disk, not delivered back to the session.
rework the Step 3 timeout fallback in the Claude and Codex revdiff skills:

- reassure the user nothing is lost and offer both paths (load or stop),
  dropping the "revdiff still open" claim they can't see after exiting
- make reading the persisted annotations mandatory on the user's reply
- fall back to the durable review history when the launcher's cleanup
  removed the temp output file
