perf(hooks): state persist contract once in serverInstructions, not per prompt (OU-198) (#162)

- drops the ~30-token static persist-contract reminder from the
  UserPromptSubmit hook injection (fired on every qualifying prompt);
  serverInstructions already states the same contract once, at session
  init ("Checkpoint after multi-step tasks; persist non-obvious
  decisions..."), matching the subagent-start hook's existing shape
  (header + rows only, no static tail).
