Add show_at:/hide_at: to Components::Column for responsive visibility

Composes d-none/d-block (bare at xs, d-{bp}-none/d-{bp}-block at sm+)
from a breakpoint pair, e.g. hide_at: :xs, show_at: :sm -> "d-none
d-sm-block" -- the same pattern already hand-written at several call
sites (observations/show/namings/{row,header}.rb,
comment_item.rb's avatar column, etc.), and a drop-in replacement for
the older BS3 .hidden-xs/.visible-xs-* utilities.

No :layout/:mode kwarg yet -- audited the codebase for breakpoint-
prefixed non-block/none display classes (d-md-flex, d-lg-inline, etc.)
and found none; only bare d-flex/d-inline with no responsive prefix.
Add one if a real case shows up.
