Fix primary key predicate planning for joins (#1943)

Restrict primary key lookup optimization to predicates that can be safely bound to the first FROM relation.

Move lookup eligibility out of the shared planner context into a planner-local candidate.
Resolve relation and positional column aliases, reject ambiguous joined-column matches,
and preserve the original WHERE predicate when a primary key access path cannot be installed safely.

Add planner unit coverage for qualified and unqualified predicates, different primary key names,
missing primary keys, alias conflicts, multiple joins, and left outer joins.
