feat(mcpx): Server.RemoveTools + client tool-list-changed hook (MC-41) (#22)

- add Server.RemoveTools wrapping go-sdk's mcp.Server.RemoveTools, mirroring the existing AddTool delegation pattern
- add ClientOptions.OnToolListChanged, mirroring the OnProgress pattern, so a consumer callback fires on notifications/tools/list_changed without leaking go-sdk request types
- needed to let a server prune stale tools mid-session and let clients react to the resulting list-changed notification
