feat(hooks): surface CLAUDE_PROJECT_DIR + prompt_id/permission_mode/effort on Common (MC-37) (#19)

Claude Code exports CLAUDE_PROJECT_DIR (authoritative project root) into every hook process's ENV, but the hook host layer never surfaced it. hooks.Common was also missing current documented CC stdin fields: prompt_id, permission_mode, effort.

- add PromptID/PermissionMode/Effort stdin fields to Common
- add Common.ProjectDir (json:"-", env-sourced) populated generically by leaf() via a commonPtr() accessor, so every payload type gets it without per-type code
