feat(cli): adopt mcpkit claude hooks seam, migrate Stop hook (#141)

- bump mcpkit pin to b64d3d5 (host/claudecode/hooks registry + provider,
  cli.CommandProvider seam)
- rename hook_stop.go -> claude_hooks_stop.go: strip cobra scaffolding,
  reshape the entry point into a hooks.Handler[hooks.StopPayload]
  (hookHandleStop); runHookStop keeps its logic byte-identical (stdin
  decode/write moved to the mcpkit chokepoint), returning hooks.Response
  instead of writing JSON directly
- add claude_hooks.go: claudeHooksProvider registers only Stop (other 6
  events port in follow-on PRs); SessionStart intentionally excluded
  (bootstrap.js stays Node - it installs this binary)
- root.go: delete hook.go/hookCmd, mount the claude tree via
  mcpkitcli.MountProviders
- plugin/hooks/hooks.json: Stop event command -> `claude hooks stop`
- re-point hook-stop tests at the new handler shape; add wire tests for
  the built `claude hooks stop` command and confirm `hook stop` is gone
