feat(cli): add CommandProvider seam + MountProviders, fix subcommand-depth doc (#10)

- add CommandProvider interface and MountProviders(root, providers...) so
  adapters/capabilities can register a uniform command tree instead of
  hand-wiring AddCommand
- add HostNamespaceCmd(name, subs...) for building a run-less host
  namespace parent (e.g. `claude`) from a set of subcommands
- fix Server.Subcommands doc and the cli package doc: cobra's AddCommand
  nests freely at any depth; the "one-level Subcommands" claim was false
- regenerate cli/README.md via `make docs`
