refactor(node-runtime): adopt tagged errors across RPC stack

Replace plain Error throws and ad-hoc JsonRpcError construction with
Effect Data.TaggedError classes and named constructors. JsonRpcError is
now a tagged error with object payload; add helpers for parseRequestFrame
(invalid request, invalid payload JSON), methodNotFound, parseError, and
fromJsonRpcErrorObject.

Introduce domain-specific error modules for proto frame codec, host
startup/readiness, indexer CLI, and unknown-cause normalization via
causeToError. Wire namespace imports through dispatcher, proto loop,
policy, runtime, and host paths so failures stay typed and consistent.

Add parseRequestFrame error tests and update existing RPC/policy tests
for the new error API.
