feat: $N intra-batch epic back-reference for backlog writes (#133)

- enables one-call epic populate + re-parent: a batch entry's epic
  field may be "$N", resolving to the item created/updated by
  entries[N] earlier in the same write, so a child can name its
  not-yet-created (server-assigned) epic parent
- works in both the create and update paths (update = re-parent an
  existing item onto a new same-batch epic)
- rejects malformed, forward, self, and out-of-range references; a
  resolution error aborts the whole batch (shared transaction from
  the prior PR)
