feat: serve MCP over streamable-HTTP via App.HTTPHandler (MC-5) (#3)

- mcpx/http.go: HTTPHandler + HTTPOption (WithStateless, WithJSONResponse),
  wrapping mcp.NewStreamableHTTPHandler behind the mcpx seam
- mcpkit.go: App.HTTPHandler delegate, additive, zero-config path untouched
- examples/http: co-mount demo proving the handler is bare and path-agnostic
- e2e HTTP round-trip tests (default, stateless, JSON-response) in mcpx/http_test.go
