feat(plugin): agents auto-file ouroboros tool bugs they encounter (#122)

- backlog-manager: files genuine ouroboros tool/server malfunctions via
  its existing `backlog` write tool before finishing, dedup via search
  first, terse item format, mentions filed id in report
- knowledge-explorer: read-only, no backlog write tool and CLI has no
  item-create command, so it surfaces a ready-to-file bug block at the
  top of its report instead of self-filing
- both scoped to genuine malfunctions only (silently-wrong/unfiltered
  results, crashes, validation surprises, wrong-project resolution) —
  never empty results, user error, or expected behavior
