fix: enforce board-operator confirm-list covers all destructive tools (#60)

- add flash_external to board-operator.md's confirm-before-destructive list — it runs an unbounded external command (platformio/esptool/make/avrdude) that can flash/erase/brick the board and was missing from the confirm-gate despite being classified RiskDestructive
- backtick esp_flash explicitly in the bootloader/partition-table bullet so it's machine-checkable alongside the others
- wrap the list in confirm-list:start/end HTML comment markers so it can be scoped robustly for testing
- add TestBoardOperatorConfirmListCoversDestructiveTools (internal/mcpserver/confirm_list_test.go), an anti-drift guard that asserts every RiskDestructive tool in the toolRiskClass registry is named in the confirm-list section — catches this exact class of drift in CI going forward
