fix: derive MCP tool annotations from the risk registry (#59)

- add riskAnnotationOpts + newTool wrapper in risk.go, deriving
  readOnly/destructive hints from the BR-71 toolRiskClass registry
- replace all 22 mcp.NewTool( registration sites with newTool( so every
  tool gets registry-derived annotations, none hand-typed
- closes the destructiveHint zero-value default gap: mcp.NewTool leaves
  ReadOnlyHint:false, DestructiveHint:true unless set, so every
  previously-unannotated tool (including read-only ones like esp_info)
  mis-signaled destructiveHint:true to MCP clients
- add TestToolAnnotationsMatchRiskClass parity test asserting every
  registered tool's emitted annotations match its risk class
