fix(plugin): persist hooks never block compaction; tighten over-eager Stop nudge (#103)

- pre-compact.js: remove both {decision:'block'} write paths (heuristic and
  precise kb-block diff); user-initiated /compact must never be hard-blocked.
  Both paths now log advisory-only observability events and always exit 0.
- lib.js DECISION_PATTERNS: drop incidental topic-word patterns (architecture,
  approach:, rationale, trade-off) that fired the Stop nudge on ordinary
  technical summaries; keep only strong decision-made phrasings (decided to,
  chose X over Y, design decision, going with, we'll use, instead of...because).
- Update pre-compact/stop tests for the new advisory-only/tighter-pattern
  behavior; add coverage for topic-words-only (no nudge) and design-decision
  phrasing (nudge fires).
