feat: add find-references support via cross-file glob scan

Implements textDocument/references by walking the AST for functions,
module/local variables, and namespace prefixes. Local bindings are
resolved scope-aware and searched within the current file only; module-
level functions and variables are searched across every file matched by
the project's lsp-config.xq glob.
