feat: add new_component approval fence (grow-by-approval guardrail) (#792)

- new fence family new_component: scans components/<name>/ directories,
  fails on any net-new component not already in the baseline
- --approve <component> flag on the fence command: the one sanctioned
  additive baseline path, narrowly scoped to new_component only;
  --update-baseline stays shrink-only for every family unchanged
- seeded baseline with all 125 current components
- tests for scan, approve, mutual-exclusivity, prune-on-removal, and the
  shrink-only invariant under --update-baseline
- docs: bbtool README new_component section + CLAUDE.md component-creation
  pointer to the enforcing fence
