feat: single-script Node self-heal and installer, drop install.sh (#41)

- self-heal.js is now the sole SessionStart hook: installs the pogo
  binary (dev binary > local Homebrew > GitHub release, pure Node,
  builtins only) and validates plugin files (binary present, every
  hook script on disk, statusLine target if configured), repairing
  the binary when missing/broken. Single SessionStart entry — no
  parallel install.sh + self-heal.js hook race.
- GitHub release install follows HTTP redirects, verifies SHA256,
  and extracts tar.gz with an in-house ustar reader (no unzip, no
  npm deps) — fully fail-open with an offline fallback that keeps
  the existing binary.
- .goreleaser.yml: darwin now produces tar.gz like every other
  platform (dropped the zip format_override) so extraction stays
  pure Node everywhere.
- hooks.json, tests, and docs updated for the single-script model.
