feat: configurable statusline visibility mode (POGOPIN_STATUSLINE_MODE) (#40)

- add POGOPIN_STATUSLINE_MODE with always (default, unchanged)/ports-only/
  fresh-only (30s) modes to plugin/scripts/statusline.js
- extend status-lib.readLivePorts with an additive, optional
  maxAgeSeconds opt and per-port updated_at passthrough; default call
  (no opts) preserves the existing 45s staleness window used by the
  BR-31 pre-tool-port-check hook
- document POGOPIN_STATUSLINE_MODE and POGOPIN_STATUS_DIR in
  plugin/README.md and CLAUDE.md
- cover all three modes plus the new status-lib opt in tests
