docs: refresh user-facing docs for MBI-003 Python API + cleanup

- README + quickstart: add "Python API (recommended)" sections documenting
  yoker.process/do/agent/session/run_sync as the primary library entry points;
  keep the low-level Agent + UIBridge pattern as "advanced" beneath it
- replace removed add_event_handler with on_event (README, quickstart x2, api/index)
- quickstart Session Persistence: rewrite away from deleted
  PersistenceContextManager -> yoker.session(id=..., persist=True) facade
  (with a lower-level Persisted(...) note)
- drop removed max_recursion_depth bullet (quickstart) and config lines
  (examples/yoker.toml x2)
- docs/api/index: fix automodule target yoker.agent (deleted module) ->
  yoker.api + yoker.core + yoker.session; add reference sections for the
  MBI-003 public API surface
- examples/plugins/demo/README: rewrite Programmatic Loading to
  load_plugins generator + registry register_plugin_* methods (deleted
  register_tools/register_skills free functions)
- Architecture: Agent layer module yoker.agent -> yoker.core
