read/cfi: validate eh_frame_hdr fde_count against table length (#897)

Truncate the binary search table to fde_count entries in
EhFrameHdr::parse, rejecting a fde_count that overflows or exceeds the
available data. This bounds the later offset computations in EhHdrTable
so they can no longer overflow. Also make EhHdrTableIter::nth return
Ok(None) for an out of range index.
