docs(era-book): completeness audit — the reader tiers are ~15% / 0%, not complete

Honest damage report against John's standard (long form carries every item;
one-pager names every item; an incomplete list is worse than nothing).

Findings:
- One-pagers name 0 items individually (prose summaries naming ~clusters).
- Long-forms: 77 entries across v6-v16 vs 530 register items ~= 15% at item
  grain (worst v16 4%, v10 5%); authored at cluster grain by design.
- The item register (items/*.json, 562 substantive entries) is the only
  item-level layer and is NOT the reader-facing account of record.
- Fable's final commit stamped the tiers "complete"; they are not, by this
  standard.
- Accounting is tangled across four systems (6.0.0 batch vs 6.x series;
  W-ID prefix != major; register superset of spec-workitems; cluster vs item
  grain), so per-era counts don't reconcile without a merge.

Not lost — the substance is in the register; the reader tiers were never
rendered to item grain. Rebuilding to the standard is bounded by the register.
