docs(eras): fix stale v12 naming drift in HANDOFF header

Increment 8. The five-way-split renumber renamed the worktree/branch
...v6_to_v12 -> ...v6_to_v16, but HANDOFF's header still described the current
worktree/branch as v12 (title, worktree/branch line, "this branch is free"
line). Corrected all three to v16, with a note recording the original scaffold
names and the 5.x->v16 program scope.

Left as-is on purpose: INDEX increment-1 keeps the v12 names as true history
(that's what Fable created); increment-3's "still shows old names" flag now
points at this fix. Legitimate references to the v12 "Proven Machine" era are
untouched.
