feat: fire bb_cache reactive on_register/on_remove triad (B1-592 A2) (#724)

Wire up the observer triad's on_register/on_remove callbacks, previously
accepted and stored but never invoked. Adds bb_cache_reactive_register()
(fires on_register on first-time registration only, not idempotent
re-registration) and bb_cache_reactive_delete() (captures the key before
the free so on_remove always gets a stable string, fires after the entry
is torn down). Both mirror fire_on_change's snapshot-then-notify shape.
