docs(eras): INDEX resume-point correction — projection A+B filed; drain is next

Increment 3: fresh-session orient + reconcile. Projection A+B verified live
(fsl#1401-1411,#1418 umbrellas; fsl#1412-1417 decision records), all milestone-
pinned. jssm-drain ledger reconciled against the live tracker (106 open, 100%
covered; snapshot 107->106, no orphan). Records the v12->v16 branch rename and
that the successor queue's step 1 was already done, so resume point = WP-8 drain.
Flags the section-C/section-D drafting gate before the drain can file twins.
