feat(era-book): per-era Mermaid dependency graphs in the generator

era_book_html.cjs now emits one Mermaid graph per version, built from
the items' own deps/blocks fields: intra-era edges solid, cross-era
edges drawn to a dashed ghost node labelled with the target's era.
Versions with no edges (e.g. 5.x) render no graph. Mermaid loads
client-side from a CDN module so the page stays a single self-
contained file. Verified by render: 11 graphs, ghost nodes correct.
