docs(era-book): items slice — v16 Public, 10 files (W-item spine COMPLETE)

The ecosystem era: the loop-first registry + behavioral/typeclass
search (W12.1,2), guardrails-as-product via mcp --policy (W12.3),
fleet-wide query governance + semantic patches (W12.4,5), the
infer/synth aspirational bookends (W12.6,7), the curriculum + eval
corpus at scale (W12.8), and machines.txt + the standing distribution
program (W12.9,10).

This completes the entire W-item spine: W6.*-W12.* across all twelve
versions plus every WP program topic (271 items). Remaining per the
plan: the e1-e7 non-W-twin tracker-row residue as a subagent sweep,
then the per-era Mermaid diagram in the generator.
