Fail fast on a shared secondary key

IndexedCache is a unique (many-to-one) index, so two values sharing a
secondary key is user error. Previously put() silently overwrote the
shared key, so evicting the stale entry wiped the live entry's index.
Detect it in put() and throw before mutating, turning silent corruption
into a loud IllegalStateException.
