docs: plan Phase 5 — own sans-io H2 core, H2-downstream/H1-upstream first

Close the §6 open question: roll our own HTTP/2 rather than FFI nghttp2
(its allocator hook routes allocations but does not bound them; our H2
limits must be reserved constants advertised via SETTINGS and enforced).

Expand the §7 Phase 5 entry into five gated slices: frame codec, HPACK
with a fixed dynamic table, bounded stream state machines, H1 translation
where the advertised stream window is the relay-buffer size (flow control
= the existing backpressure model), then ALPN/GOAWAY/sim/bench
integration. H2 upstream multiplexing deferred to a later slice.
