docs: compress DESIGN.md to decisions taken, results measured, lessons learned

The document had grown by accretion: option surveys for choices long since
made, slice-by-slice implementation logs, a proposed module layout that
CLAUDE.md's real module table superseded, and a references section whose
links resolved nowhere.

Now: §1 guiding decisions (all six survived contact), §2 the completion-
callback I/O model plus the war stories (shutdown-before-close, one
completion per overlappable op, the cached clock), §3 what the 0.16 std
landscape ruled out, §4 memory (including the cache-line isolation lessons
with their HITM numbers), §5 data path, §6 TLS/kTLS as built (rejected
options reduced to one-line whys), §7 build history with per-phase measured
results and a consolidated deferred-improvements list (io_uring flags et al),
§8 references with verified URLs.

Section numbers cited from code comments (§2, §4, §5, §6 "kTLS design",
§7 Phase N, "I/O architecture") are preserved; the one stale cite
(proxy.zig pointed backpressure at §8, the old module layout) now points
at §1.4.
