docs: comprehensive documentation review and refactoring

Cross-checked all documentation against actual implementation and
corrected hallucinated content, outdated references, and missing
feature documentation.

Key changes:
- Fixed incorrect BaseUIHandler references (class does not exist)
- Corrected UIHandler.start() signature (takes agent, not model/version/config)
- Fixed Agent constructor examples (no model parameter)
- Corrected tool names (websearch/webfetch, not web_search/web_fetch)
- Updated to v0.5.0 with actual feature list
- Replaced non-existent model names with actual curated defaults
- Added multi-provider architecture documentation
- Added bootstrap wizard documentation
- Documented provider configs (Ollama, OpenAI, Anthropic, Gemini, Generic)
- Documented NetworkError user-friendly vs debug messages
- Documented security: masked API keys, chmod 600, no history logging
- Marked keyring integration as PLANNED (TODO S.1)
- Added thinking mode 'silent' to slash command tables
- Created new docs/models.md page from curated_models in providers.py
- Fixed stale references in demos/README.md
- Updated CLAUDE.md module structure to match actual codebase
- Updated dependencies list in installation.md
- Added gemini guide to docs/index.md toctree
