Fix TEI paragraph split when a paragraph starts after a trailing reference marker (#1482)

Paragraph boundaries in TEI were being lost when a paragraph ended with a reference callout and the next paragraph began immediately after it. The decoding/serialization path treated marker-adjacent paragraph clusters as continuations, collapsing distinct paragraphs.
