Automate GitHub releases and refresh the unreleased changelog

Add a workflow that creates a GitHub release from the matching CHANGELOG.md
section when a release tag is pushed, falling back to auto-generated notes
when no section exists. Update the [Unreleased] changelog to cover all
changes since 0.9.0.
