fix(api): persist org Meta on creation (#555)

Both create paths built the db.Organization without copying the request's
Meta map, so any meta sent at creation (e.g. meta.name, the org's display
name) was silently dropped and every org persisted with empty meta.

Copy req.Meta / orgInfo.Meta into the db row in createManagedOrganizationHandler
and createOrganizationHandler so a provided name (and any other meta) survives.
The integrator portal's managed-orgs list reads meta.name for its primary label.
