chore(tools): Add `count` parameter to the `plan` tool (#799)

The `plan` tool's `next` and `prev` actions now accept an optional
`count` argument, letting the assistant advance or rewind multiple steps
in a single call instead of issuing one call per task.

When `count` is omitted it defaults to 1, keeping existing behavior
unchanged. Passing `count: 0` is rejected with a clear error message.
For `prev`, a `count` that exceeds the number of completed tasks still
discards the plan, now consistently regardless of the exact value.

The tool output header now names the action and includes the step count
when more than one step was moved, e.g. `Advancing plan
"refactor-config" by 2 steps (2/5 complete)`. The `parameters` display
style is also switched to `"off"` since the argument- formatting path
was removed.
