Re-record shared fixture cassettes live on vcr-rewrite (#351) (#352)

`vcr_fixture` downgraded record-mode `rewrite` to `new_episodes`, so a
full-suite `vcr-rewrite` replayed the already-normalised shared fixture
cassettes for every module after the first. The replayed interactions
carry the workspace-portable placeholder ids, which 404 the live calls
of later modules (~66 failures in an 8-module run).

Use `all` mode instead, so each module re-records the shared fixtures
live with real ids. Deterministic fixtures (e.g. `search('Tests')`)
re-record identically, keeping the shared cassettes consistent.
