feat: surface the reason when ralphex refuses to run

running ralphex without a plan on a non-default branch returned a bare
"no plans found" error, with no hint that interactive plan creation was
suppressed because of the current branch.

`tryAutoPlanMode` now returns an explanatory error for every suppressed
case (non-default branch, branch-detection failure, and the review /
external / codex / tasks-only modes), while still wrapping
`ErrNoPlansFound` so existing callers stay unaffected.

other precondition failures also got actionable hints: missing `.git`,
`claude`/`codex` not in PATH, and the `--init` repo-root checks.
