[#20] Wire dashboard map filters to cross-form scope and option colors

- useMapByParent / DashboardMapHeader / index / MapPopupCard: always
  resolve filter values and legend/marker colors cross-form, scoped by
  source_form_id (sent as parent_form_id); colors derive from
  QuestionOption.color via resolveColorMap
- useMapFilters: drop the dead monitoring_form_id lookup so the
  'monitored last year' toggle is cross-form (any monitoring child)
- Remove the obsolete single-form getQuestionOptions helper
- Configs: remove form_id from map filters and strip color_map entries
  that just duplicate the option color, keeping only genuine overrides
  (final_recommendations, not_active) and _no_info
