[FIXED] Avoid panic in PullSubscribe consumer create path (#2088)

* fix: avoid panic in PullSubscribe consumer create path

Use the durable consumer name when recovering from consumer-already-
exists errors, guard against nil consumer info before dereferencing,
and remove inner info variable shadowing in subscribe().

Fixes #1258

* fix: remove redundant PullSubscribe nil guards
