feat: auto-detect TLS in setup() from endpoint port

When insecure is not explicitly passed, setup() now infers it from the
endpoint: port 443 → insecure=False (TLS), anything else → insecure=True
(plaintext). Explicit insecure=True/False still takes precedence.

This means pipelines pointing at herald.chimefrb.xyz:443 get TLS
automatically with no code changes — only the endpoint env var needs
updating.
