fix(modtools_chat): show member name not group name after group-change + standard message (#787)

When a member has firstname/lastname but no fullname, the ModTools chat
list was showing "GroupName Volunteers" instead of the member's name.
The User2Mod SQL was aliasing fullname as firstname and hardcoding
lastname='', so when fullname IS NULL the naming logic fell through to
the group-name branch. Fix fetches real firstname/lastname columns from
users and adds a fallback naming branch that uses them.
