expand to all Google Maps API failure surfaces

Generalize `showGeocodeError` → `showGoogleMapsError(label, error)`
and route every silent catch through it:

- Maps JS API loader catch (was `console.error`)
- `window.gm_authFailure` — Google's global called on any Maps JS
  auth failure (bad key, wrong referer, billing disabled). Installed
  once per page load via a `window._mo_gmauth_installed` guard.
- Geocoding API catches (both place→latlng and latlng→place)
- Elevation API status handler (was an empty else branch)

Slot renamed `#geocode_flash` → `#gmaps_flash` since it now handles
loader / auth / elevation failures too.
