docs: adopt Keep a Changelog 1.1.0 (#681)

Add a root CHANGELOG.md in Keep a Changelog 1.1.0 + SemVer format, seeded
with the user-facing changes merged since v3.0.1. Document the practice in
CLAUDE.md: every user-facing PR appends to [Unreleased] (grouped
Added/Changed/Deprecated/Removed/Fixed/Security), internal-only work is
exempt, and the release step renames [Unreleased] to the version + date.
